Application form SC-1 for the San Francisco International Airport (SFO) Second Chance Noise Insulation Initiative, administered by C. Kell-Smith & Associates, Inc. Targets residential property owners who declined or did not respond to prior Noise Insulation Program (NIP) phases. Eligibility requires location within the 65-dB CNEL noise contour per the SFO Noise Exposure Map (FAA-accepted), structure built before October 1, 1998, site visit confirmation, acoustical testing in habitable rooms, and grant of a perpetual avigation easement to SFO and the City & County of San Francisco. Form collects contact/mailing information, property address, number of windows/doors/floors/dwelling units, existing A/C type (central, wall mounted, thru-wall, thru-window), unpermitted renovations, known building code issues, and prior NIP invitation history.
