Exhibit 386 (AR 021699–021701), handwritten field notes from a site visit dated 1/28/01 at Ace Airport View, Des Moines, documenting DM1 Monitoring Station observations. Notes record SDS 1 outfall in SW associated wetlands, steep sides, turbid water with boulders/stones, Old Boom present, water turbid from outfall significantly more so than ambient water at 11:45 AM. Second page records SW pond outfall instrumentation gone, SW pond adjacent to creek draining from creek, water draining to grated inlet, separation of SW pond from creek, no photos yet. Third page references comment letter on embankment flows, foster/hopper attenuation, non-point discharge locations for flows, and clogging of material.
